The Utah State Bar uses a photo-based ID card for members of the Utah State Bar.
Please complete the form to request a new Utah State Bar card.
Delivery Options:
Option 1 Pickup In Person:
If you are in the Salt Lake area, you can come into the Bar offices between 8 AM and 5 PM Monday – Friday to have a card created for you. The process takes about 10 minutes and requires a picture to be taken.
(Please note that any cards marked for pick up in person, if not picked up after a week will be mailed out.)
Option 2 By Mail:
If you choose it to be mailed, it will be mailed to the address you have on file with the Utah State Bar.
