Request a Bar Card

The Utah State Bar uses a photo-based ID card for members of the Utah State Bar. 

There are two ways to obtain a new Utah State Bar Card. 

Option 1 Walk -In:

If you are in the Salt Lake area, you can come into the Bar offices between 8 AM and 5 PM Monday – Friday to have a card created for you. The process takes about 10 minutes and requires a picture to be taken.

Option 2 By Mail or Pick up:

Please complete the brief form below to have a new Utah State Bar card mailed or for pick up at our office for you.
