All mentor applications are reviewed by the Utah State Bar Committee on New Lawyer Training. The Committee requires the submission of at least three references from the legal community with whom you have had professional dealings within the past three years. Essentially, we would like you to provide the names of attorneys who are familiar with your demeanor and general reputation in the legal community Please follow the following guidelines when choosing your references:

• Only one (1) reference can be from your current firm or legal employer.
• At least one (1) reference must be an adverse/opposing counsel.
• If applicable, provide one (1) judicial reference from a judge familiar with your performance and demeanor in court. If you do not have a judicial reference or “adverse/opposing counsel”, please provide the names of attorneys with whom you maintain a professional relationship. Examples of acceptable references include, but are not limited to, outside counsel (current or previous) representing your organization, attorneys who have appeared adverse to you in some capacity (contract negotiations, mediations, etc.), or attorneys at government entities with whom you have professional dealings. Mentor Acknowledgment:

• I wish to serve as a mentor in the New Lawyer Training Program in accordance with its rules and regulations.
• I acknowledge that the information provided in this application is complete and accurate to the best of my knowledge.
• I certify that I meet all of the mentor requirements I checked above.
• I understand that my participation in the NLTP is contingent upon the approval of my application by the Utah State Bar Committee on New Lawyer Training.
• I acknowledge that I will be awarded twelve hours of CLE credit only if I satisfactorily complete all program requirements.
• I agree to attend or view the Mentor Training and Orientation program.
• I agree to make the necessary time commitment to maintain a mentoring relationship. If my work or life circumstances change so that I cannot contribute at this time, I will contact the NLTP Coordinator.
• If, after I become an approved NLTP mentor, I become formally sanctioned in any jurisdiction, I will notify the NLTP Coordinator and may be taken off of the approved mentor list.
• Pursuant to Rule 14-515(a)(1) of the Rules of Lawyer Discipline and Disability, I waive confidentiality with respect to allowing the Office of Professional Conduct to provide a complete report on my grievance history to the Utah State Bar Committee on New Lawyer Training in connection with my NLTP mentor application.
• I understand that all mentor appointments are subject to reassessment, if necessary, in the future.
